Incoming Student Checklist:

  • Parent or guardian must be a legal resident of Milford, NH (documentation such as lease, phone bill, electric bill is required).
     
  • Proof of a health exam physical within the last year or proof of a scheduled physical (including date and physician).
     
  • Immunization records required before the student may attend school.
     
  • Copy of the student's birth certificate.
     
  • Current academic records from previous school. (Unofficial copy of transcript of grades and/or last report card.)
     
  • Grades to date of withdrawal when transferring during the school year.
     
  • IEP & most recent Educational Evaluation when applicable.
     
  • Proof of immigration status (if student is not a US citizen and/or is transferring from a foreign country).
